How to Register

Important
Every care has been taken in the preparation of information on this Postcode Mapping Tool, however there may be instances where the Postcode Mapping Tool does not include your location even though it is within our Practice Boundary. An example of this may be new housing developments where the postcodes are not yet included on Google.

Complete a New Patient Registration Form
If you live within the boundary area, please complete our new patient registration
When you register, it's helpful to have your NHS number. You can use the NHS website to find your NHS number.
Online Access to Medical Records
You can register for access to your medical records by the following
- Downloading and registering via the NHS app
- If you are unable to complete the online registration via the NHS app, please visit the surgery in person to complete an online access registration form and bring photo ID and recent proof of address for verification.
- Suitable forms of photo ID are driving licence or passport. Proof of address must be an official letter such as bank statement, council tax/utility statement.
- For proxy access, both for children and older patients, all parties need to visit the surgery together with their ID for verification.
- If proxy access is requested for a 'looked after' child, we would also need to see some evidence of this.
- If you do not have any photo ID, a clinician may be able to vouch for you if you have recently been treated by the surgery.
- Suitable forms of photo ID are driving licence or passport. Proof of address must be an official letter such as bank statement, council tax/utility statement.
Temporary Registration
If you are ill while away from home or if you are not registered with a Doctor but need to see one, you can receive emergency treatment from the local GP Practice for 14 days.
Temporary Services
After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local Practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that Practice.
To register as a temporary patient simply contact the practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a Practice in the town or area where you are already registered.

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register.
Immigration status or nationality don’t matter - reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved.
Your Named GP
All patients who are registered with the surgery will have an allocated named GP who will be responsible for your overall care. However, it may not always be possible or appropriate for you to see your named GP when you visit. If you have a preference for a male or female GP, the practice will make reasonable efforts to accommodate your requests. If you wish to know which GP has been allocated to you, please ask reception at your next visit.
